HIPAA Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Summit’s Healthcare, LLC is legally required by the Health Insurance Portability and Accountability Act (HIPAA) to maintain the privacy and confidentiality of your Protected Health Information (PHI).

  3. For Healthcare Operations: Reviewing treatment outcomes, training medical staff, optimizing our telehealth delivery systems through PracticeQ, and conducting internal audits.

Disclosures Requiring Your Explicit Authorization

We will not use or disclose your PHI for marketing campaigns, sell your health data, or share psychotherapy/counseling records without obtaining your signed, explicit written authorization. You reserve the right to revoke this authorization in writing at any moment.

Your Health Information Rights

Under HIPAA regulations, you retain the following rights regarding your medical records:

  • Right to Access: You can request electronic or physical copies of your medical records and billing files.
  • Right to Amend: You can request a formal amendment to your medical files if you believe the clinical information is inaccurate or incomplete.
  • Right to Restriction: You can request that we restrict how we use or share your PHI for treatment, payment, or general operations.
  • Right to Accounting of Disclosures: You can request a historical log detailing the dates and reasons your PHI was shared with outside entities over the past six years.
  • Right to Confidential Communications: You can request that we contact you only via specific channels (e.g., via a designated personal cell phone number or physical mail).

Data Security Updates

If an unexpected incident occurs that compromises the privacy or security of your health information, we will notify you directly right away in accordance with federal timelines.

Complaints and Contact Information

If you believe your privacy rights have been violated, you can file a formal complaint directly with our clinic’s privacy officer or with the U.S. Department of Health and Human Services Office for Civil Rights. Summit’s Healthcare, LLC will not retaliate against you for filing a complaint.
